Naib Subedar Madan Lai was a member of a commando team detailed for search and destroy operation in Alampil Forest during Operation CHECKMATE IV in Sri Lanka. The operation commenced on 16August 1988. Naib Subedar Madan Lal was bringing up the rear of the team with his troops. He heard a distant noise. The team commander asked him to take a patrol of 10 other ranks and to go to investigate the noise. Naib Subedar Madan Lal having gone 150 metres to the right noticed a green tent of militants. He quickly crawled up to the tent, simultaneously giving orders to his patrol to fan out and then fired two bursts from his AK-47 into the tent.

However, when Naib Subedar Madan Lal was changing his magazine, he was fired upon simultaneously from two trenches. Though fatally wounded, he managed to fire his second magazine also towards a trench and hit a militant. He later succumbed to his injuries. In this action, three militants were killed and six to seven wounded, mostly by Naib Subedar Madan Lai's fire. Naib Subedar Madan Lai displayed conspicuous courage and valour in action against the militants.
